health insurance

Deductible

A portion of the covered expenses (typically $1000, $2500 or $3500) that an insured individual must pay before benefits are paid by the insurance plan. Deductibles are standard in many PPO policies and are usually based on a calendar year.

Co Insurance

Your share of the costs of a covered health care service, calculated as a percentage (for example, 20%) of the allowed amount for the service. You pay coinsurance plus any deductibles you owe. For example, if the health insurance or plan's allowed amount for an office visit is $100 and you've met your deductible, your coinsurance payment of 20% would be $20. The health insurance or plan pays the rest of the allowed amount.

Out of Pocket Maximum

This is the total you have to pay each year for most of your covered services. It does not include your premiums. Each family member has a yearly out-of-pocket maximum and there may be a family out-of-pocket-maximum also. When an individual or family reaches the maximum, they do not have to pay most out-of-pocket costs for the rest of the year.

Co-Pay

A co-payment or copay is the amount you as an insured person pay for an expense when you visit a doctor, laboratory, ambulatory care center, hospital, or pharmacy. It's a set amount of money for a certain kind of provider. For example, if you see a primary care doctor, it may cost you a $25 copay. if you see a specialist, it may cost you a $50 copay. Each time you see that doctor for your health care, you'll pay the same copay at the time of service.

Dental

Great oral health is an essential part of a healthy lifestyle. Our individual dental insurance plans can help you maximize your oral health. In fact, people with dental insurance are more than twice as likely to visit their dentist regularly.

Choosing a dental insurance plan can be overwhelming—monthly premiums, annual maximums, deductibles and more leave you with a lot to consider. BFG wants to make your decision easier by demystifying the process of selecting a dental insurance plan.

Vision

Why is vision health important?

You only get one set of eyes. Take care of them.

Your eyes not only affect how you see, but how you feel. Caring for your vision can lead to a better quality of life, including:

  • Fewer headaches
  • Better performance at school
  • Less computer eye strain
  • More effectiveness at work
  • Better performance in sports

It all starts with an eye exam.
